Notes:

A risk assessment should be carried out for each dancing venue.

Take ONE aspect/item under each heading,  so one item will be recorded in isolation.  As an example in the template there are two entries in the column 'What are the Hazards' under the heading 'Dancing space - is it suitable?'  It may be that several individual items, each needing a separate row,  will need to be included under each heading.

The template is in MS Word and Adobe PDF  so add items/rows if required.  Don't delete items under the column heading 'What are the hazards?'  If they are not needed just record as N/A.  Extra headings under this column can be added if required.

We encourage sides to use this template to demonstrate to our insurers - in the first instance - that we have not negated the insurance policy.
