Any and all personal information held by Nemesis Works for those on our mailing is considered confidential and will not be shared with any third parties.

To unsubscribe from our mailing simply email "LEAVE" to nemesisworks.yvonne@gmail.com and you will be removed from the list and all contact information will be deleted.

PILATES AND THAI YOGA MASSAGE CONFIDENTIAL HEALTH FORMS

Please be aware before taking part in any Pilates classes or Thai Yoga Massage treatments you will be required to fill out a health form in order to ensure that the teacher/practitioner is aware of any health issues and is able to offer the most suitable class/treatment for individual clients.

These forms are confidential and will not be shared with any third parties except in the event of medical emergencies.

All health forms will be kept for no longer than 10 years after the client's final class or treatment. During that period all information will be kept secure and confidential. After this time all paper forms will be shredded and disposed of in a secure manner and any digital information will be deleted.
