  • Step 1 Click link Report the death of an annuitant to access OPM page

    • OPM page will open "My Annuity Benefits" Life Events ...Reporting a Death

  • Step 2 Click one of the 2 links at the bottom of the OPM page that look like:

    • Report the Death of a Retiree or Survivor Annuitant or

    • Report the Death of a Federal employee

OPM page "Report Annuitant Death" will open

• • Step 3 click on -Continue on the bottom of the OPM page

    • A new OPM page "Report Annuitant Death" submission form will open.
