If you reside within these borders and would like to join right away, please print, fill out, and return this Liability Form to Ashley via e-mail at momsclubmg@gmail.com or by mail. Another option is to fill out an online Liability Waiver here. Then submit your $25 annual dues via PayPal or a check, as written in the linked physical form.

If you would like to join the Maple Grove MOMS Club and do not reside in Maple Grove, Osseo, or Corcoran, and your city does not have a chapter for you to join, please also fill out this Out-Of-Area-Request Form and submit it along with your Liability Form.
