The following are MANDATORY FORMS (UPDATED September 2022) to file a Petition to Approve a Minor's Contract or a Petition for an Early Release of Coogan Funds pursuant to California Family Code § 6750 et seq. or California Labor Code § 1700.37

If there are two paying entities, file two petitions.  However, if the second paying entity does not have jurisdiction to file a petition, you may interlineate the petition and proposed order.  Click here: Sample Petition and Proposed Order for Multiple Paying Entities.

You will also need to attach a Family Law Case Cover Sheet to the petition.  Important: Check the box for Petition – Minor’s Contract (FC § 6751) [4047].

Commencing November 15, 2021, all petitions to approve a minor's contract filed in Los Angeles County must be filed electronically through an approved Electronic Filing Service Provider.  See Public Notice and General Order:  EFILINGINFAMILYLAW.pdf (lacourt.org)  Filing fee is $435.00.

The court will sign your order electronically.  Therefore, please file a proposed order along with your petition.  You no longer need to provide hardcopies of the order or a return envelope.

Material to Unblock Funds

You will need Adobe Acrobat Reader to view and print the forms from the web.  If you do not have Adobe Acrobat Reader, you may download the application for free from Adobe's Web Site.  

For the safety of the minors, please black out the street address where the minor resides in any document filed with the court.

  For Release Of Coogan Funds Before Minor Turns 18:

Instructions and Petition For Withdrawal Of Funds From Blocked Account:  Use this form to request a release of  funds from a Coogan Account before the minor attains the age of 18 or becomes emancipated.  Note that you must click on each of the links above to obtain both documents. You will also need Family Law Case Cover Sheet, Declaration Form, Income and Expense Form, and Order For Withdrawal Of Funds From Blocked Account. For the safety of the minor, please black out the address of the minor on any documents you submit to the court, e.g., bank statement, copy of driver's license, etc.

  For Release Of Coogan Funds After Minor Turns 18:

No court order is required to terminate the blocked account.  "Upon reaching the age of 18 years, the beneficiary may withdraw the funds on deposit in trust only after providing a certified copy of the beneficiary's birth certificate to the financial institution where the trust is located."  (Family Code Section 6753(b).)
