All forms (Commitment form, Content and information form, permission slip, iPad usage form, and acknowledgement of fees form) are due by Tuesday August 21 @ 6:30pm. Students and parents not turning in forms will result in student being dropped from the program and lose all performance, trip, and other incentive opportunities.
