Here's a link to the 20-item Morality is Founded on Divine Authority scale as reported in Piazza and Landy (2013), JDM. This scale assesses people's meta-ethical belief that God (and not human thought) is the source of moral knowledge.
We now have a 5-item shortened version of the scale with equally strong psychometric properties:
1. Everything we need to know about living a moral life God has revealed to us.
2. What is morally good and right is what God says is good and right.
3. If you want to know how to live a moral life you should look to God.
4. Acts that are immoral are immoral because God forbids them.
5. It is possible to live a righteous life without knowledge of God's laws. [reverse scored]
