  1. Schedule your first appointment, for either of the following:

  • Any current health care needs, such as an annual physical, illness, allergies, minor injury, chronic condition, and more

-or-

  • A new patient appointment if you do not have any specific needs but would like to meet with a provider to establish care


  1. Bring the following to your first appointment:

  • Your driver’s license and Blue KC member insurance card

  • Your current prescription list

  • Name and contact information of PCP (optional) - if you have an outside provider and would like to share your visit information, the Clinic can send a full summary with your authorization

Examples of why you might establish care with the ISD Employee Health Clinic:

  • You do not have a primary care provider and would like the ISD Employee Health Clinic care team to meet that need

  • You have a primary care provider, and would like to take advantage of the convenience and cost of the ISD Employee Health Clinic services for minor or acute health care needs, or when you can't be seen quickly by your own provider
