The following forms should be reviewed with a parent or guardian. Then the first form can be returned to the teacher in order to participate in all class activities and receive a copy of the text and workbooks.
Activity Participation Agreement — Safety rules to be followed for all activities both in the lab and in the gym!
Document Signatures Letter — If you lost the letter you were given in class. Get this ONE page signed, not all the others
Mr. Clark's Class Expectations. — By grade 12, most of this should be well known. Especially if we have learned together in the past. When in doubt, check it out!
