Used for high level behavioral concerns
Including: Self harm & suicidal ideation, Family deaths, Threats to the safety of self or others, Confirmed substance abuse, etc.
Used for:
incidents of sex-based violence, sexual harassment, sexual assault, domestic violence, dating violence, or stalking
violations of :
Title IX Grievance Policy
Student Policy Prohibiting, Sexual Assault, Domestic Violence, Dating Violence, and Stalking

II. COMPLETE ‘BACKGROUND INFORMATION
A. Your Full Name: Provide your First Name and Last Name
B. Your Position/Title: Provide your title (eg. RA, SRA, RD)
C. Your Phone Number: Provide your cell number (xxx) xxx-xxxx
D. Your Email Address: Provide your Pride email address
E. Your Physical Address: Provide your building and full room number
F. Click the box: ‘Email me a copy of this report’
G. Nature of this Report: Choose ‘General Conduct Report’ from the dropdown menu
H. Urgency of this Report: Choose ‘Normal’ from the dropdown menu
I. Date of Incident: Provide the date by using the calendar icon (dates must always be in this format: YYYY-MM-DD)
J. Time of Incident: Choose the specific time using the dropdown menus
K. Location of Incident: Choose the location using the dropdown menu
L. Specific Location: Specify exactly where the incident took place (eg. Room 1124B, 6th floor elevator, 13th floor lounge)
III. COMPLETE ‘INVOLVED PARTIES’
A. Name or Organization: List First Name and Last Name (if organization, list full organization name)
B. DOB: Provide person’s birthday using this format YYYY-MM-DD
C. Select Gender: Choose the appropriate gender from the dropdown menu
D. Phone Number: Provide the individual’s cell number using this format (xxx) xxx-xxxx
E. Select Role: From the dropdown menu choose one of the following:
· ‘Alleged’ if the individual is to be charged with a conduct code violation;
· ‘Victim’ if the individual is someone affected by the actions of the ‘Alleged’;
· ‘Witness’ if the individual was present during the incident and/or became involved in the incident in some way.
F. Email Address: Provide the individual’s Pride email address
G. ID Number: Provide one of the following:
· HUID number for Hofstra student by typing only numbers (no dashes/no spaces)
· Leave Blank for non-students
· Full organization name if organization
H. Hall/Address: Provide full room number (including letter if suite), and building name; if commuter, Provide Local Address; if organization, leave blank
I. Click ‘Add Another Person’ and repeat III.A. à III.H. for all remaining parties involved
IV. COMPLETE ‘DESCRIPTION/NARRATIVE’ SECTION – Remember to follow all guidelines for writing incident reports, providing as much factual information as possible and omitting personal opinions/heresay.
V. COMPLETE ‘ADDITIONAL QUESTIONS’ SECTION
· Indicate whether any party received an NTA for this incident by choosing one of the following from the dropdown menu: Yes, No, Unsure
VI. COMPLETE ‘SUPPORTING DOCUMENTATION’ SECTION by uploading any additional information regarding the incident such as photos, prior emails, etc.
VII. COMPLETE ‘ONE LAST STEP’ SECTION by typing in the correct numbers/letters
