Activity Consent Form (permission slip) ~ required for all events except our regularly scheduled meetings.

The Annual Health & Medical Form ~ it's just that. It must be updated every year, Part C must be completed

for any event longer than 72 hours (Summer Camp, Expeditions etc.)

We keep 1 copy on file specifically for the Troop.

Do yourself a favor & make 2 copies because You will have to provide a copy for Summer Camp, required by Council.
