I've been in the same situation for several years now but I haven't decided which route to go. I've pretty much collected the same over the past 7 years +/- $50k. I'm solo and the big negative is my medicaid % is climbing, a lot of it is simply more families that were on private insurance are now on medicaid. The question has been if I should add chairs and an associate or start reducing medicaid. So far I've just stayed the same. I don't like the thought of having to bring in more staff and the stress of filling chairs for an associate. I also feel like eventually every kid will be on gov healthcare so I don't think contracting would be a great idea either, so I'm just in auto-pilot mode til I get a wild hair 1 way or the other. Problem is that I'm not collecting more and the cost of doing business goes up every year and you can't just bump up your fees to get a raise anymore, at least not much of one.

I did some digging on the Health and Human Services website, and the most significant one seems to be reducing conscious sedation down to 85 dollars or something ridiculous. Not from Texas, but I've seen the ups and downs of Medicaid reimbursement here in Indiana over the last 25+ years. When I first began, they paid about 75% of UCR, especially for dentures ($750-800 in the early 90's). I took over for another dentist in town at a nursing home and saw that he did dentures for EVERYONE, including patients in comas! Was a lucrative thing for him, left me with very little to do. Had this gig for about 7-8 years, did do several dentures per year till Indiana figured out that guys like the previous dentist were bilking the system for big $ and now there wasn't any left!

So guess what happened, they slashed fees to the point we couldn't do it any more. Same happened with most the dentists in Indiana, left in droves. After several years of complaints of lack of providers, they raised fees significantly. You can guess what happened again this time. Same old story, eventually slashed fees again after a couple of years and that was it for me.

So I'm sorry to say, looks like that's what is happening to you down in Texas.

Well, Medicaid nailed our state pretty hard in 2017. We can "live" with the fees, but the administrative nightmare they created is just ridiculous. This is the sign of the times. The insurance companies aren't far behind doing the same thing. People want excellent health care benefits but don't want to pay for it, so the providers reimbursement gets cut. Sign of the times, man.

I've never taken Medicaid but PPO's are sure giving us the squeeze.

I remember 10 years ago thinking my income would always creep up. Good times; good times.

We are at the Mercy of the big insurance companies. I've told my team for years that as health care changes we will work harder for less money. I am fortunate that I could get out now and be done with it all, but I certainly feel for all you guys who still have many years of practice ahead.
