Startup in Trouble

Hi

I have a start up practice one year old, we have just getting breaking even even production wise. I know I need help at this point however I cannot afford a consultant/coach . Can you guys recommend some good courses I can take.

I need help in getting new patients. I spend a lot of money on marketing which I do not think give me back much. We are breaking even with difficulty I have started taking Medicaid which helps ( 50 $ for exam x ray and prophy is better than 0$) Practice is 50% PPO at this point. So here are the issue.

Very satisfied pts but I do not have much google reviews (how do I get them)

Marketing wise done SEM, SEO and direct mail (spend around 50,000 in the first year on marketing) what else can I do. I am worried that at this point I cannot spend that much on marketing and it may decrease the flow of PPO pts.

I had hoped to get a lot of patients from surrounding business, that has not happened yet although a small number of patients have started coming from surrounding business.
