At the end of each term the lab managers will conduct an evaluation of each student's performance in the lab. While we require at least a 1 year commitment from each of our students, we hold the right to ask you not to return the following semester if we feel the experience is not mutually beneficial.

Once you finish filling out the form, please email the lab managers to let them know - this form is not regularly checked, so if you don't inform the managers, they most likely won't see the request from the form alone.
