• Cruise Booking Form:  Use this form if you are unable to complete our online cruise registration form or if you prefer to register in person or by mail.

• Credit Card Authorization Form:  This form is for Group Leaders to authorize the use of your credit card for group cruise payments and for any charges payable directly to Crafty Neighbor (not Crafty Neighbor Travel).  You will need to complete this card for each unique credit or debit card you use, including cards with new expiration dates. Cruise passengers will use the "Pay Now" button on your most recent invoice to make cruise payments.

• Group Payment Form:  This form is for use by Group Leaders reporting payments made directly to the organization to be processed by Crafty Neighbor Travel.

• Luggage Tag:  This specialty luggage tag is to be used by crafting groups on all luggage containing crafting supplies.

• Printable Pre-Cruise Checklist: Handy list to make sure you're not forgetting anything important you need to do before sailing.

• Travel Protection Waiver:  Use this form to indicate that you have elected to decline the travel protection plan offered.

• Waiver of Liability:  This is a general release waiver, required by all group cruise participants.  Online registrations acknowledge this waiver via a checkbox at the time of registration.
