All of our Family Medicine residents are required to participate in AT LEAST ONE CHART AUDIT SESSION IN EACH OF THEIR 16 WEEK FAMILY MEDICINE BLOCKS. Chart audits look at the quality of care through reviewing the documentation in a patient’s chart and are based on the CPSO’s audit form. When in your own practice, you will be required to do a self-audit as part of a mandatory QI CPSO exercise (currently every 5 years) or may even be subjected to a CPSO practice audit for various reasons. The guidelines for appropriate medical documentation can be found on the CPSO website under: “CPSO policy for Medical Records”. These can be found by using the Google search terms “cpso medical record policy” or from the "Resources" section on this site.
There are 3 audit periods
April, August and November
The actual date that the audit will occur is determined by the specific centre. Residents need only to participate. Residents are expected to do at least 2 chart audits per Family Medicine block. For Residents, please become familiar with the audit process by reviewing the Tutorials section below prior to your audit session. You can also ask your supervisor to confirm on the date the audit session will occur.
The audit is done entirely online using the "Audit Tool" that can be found by clicking the icon below (note, this will open a new page). You can also select "Tutorials" to learn how to do chart audits.
All of the Centres in the Group Audit Stream have EMRs. Unless you have another computer nearby, you will need to open another browser window to complete the online Audit Tool. Then you will need to flip back and forth between the Audit Tool and reviewing your EMR. Alternatively, you could use your laptop as an input device for the Audit Tool while reviewing the EMR on the main computer.
[Note: Although every effort has been made to fix bugs that come up using the online Audit Tool, there still exists a possibility of problems. If you see a computer error screen, either try using your back button on the browser or just close down the entire browser and start again. You should also tell your Audit Leader or Admin. Also, there may be some minor formatting issues that will be fixed in future versions.]
