We have created this resource pack to further support you in caring for your patients with a learning disability, although it may also be useful in supporting all individuals who may struggle in accessing healthcare and decision making independently (Dementia, Autism, SMI, or those in vulnerable circumstances).

This pack is intended to help you explore why arranged appointments may have been missed, and whether this was due to individuals ‘not being brought’ or whether they ‘did not attend’.

The resources in this pack have been created in collaboration with Dr Amy Lampard (Designated Doctor for Adult Safeguarding), Sheffield Mencap and Gateway, & South Yorkshire Integrated Care Board Quality Team.
