Learning Disability Health Checks Quality Project – Support Offer for Practices
Sheffield Mencap and Gateway’s Learning Disability and Autism Health Support Service, and NHS SY ICB Sheffield (Quality and Primary Care Development Nurse teams), have worked together on a new project to support practices to further develop the quality of their Learning Disability Annual Health Checks (LD AHCs).
A limited number of practices will be able to book in for a visit from Sheffield Mencap, during which Mencap will:
Use a short Quality Tool Survey to review (with the practice) a small sample of completed LD AHCs records
Offer simple, supportive advice and tips on how practices may wish to consider improvements to their LD AHCs approaches
Recognise good practice that the surgery is demonstrating, that can be shared to help other surgeries
The results from the Quality Tool reviews will also be collated anonymously and will help to inform plans for how Sheffield Mencap’s Health Support Service project can support practices in 2026/27 (for example, training and resources).
Most practices already have information sharing agreements in place with Sheffield Mencap, but if you don’t this can be easily arranged with them. Sheffield Mencap won’t take any patient information away from the practice as part of completing the Quality Tool and will only record anonymous data.
