Asthma Medication Form

This form allows your student to use their asthma medication during school hours and can be supervised by a staff member when needed.

Epinephrine (Epi Pen) Medication Form

This form allows your student to use their epinephrine pen in school when there is an exposure to an allergen and be assisted by designated staff member when needed.

Consent to Individual/Group Counseling Services

This form allows for your student to participate in individual or group counseling with a clinical SFUSD staff member or mental health intern.

Authorization to Release of Confidential Information (ROI)

This document allows the school administrators to communicate directly with a provider or outside organization regarding a student. This can help provide better support for the student while in school.
