Work Permit General Information
In Pennsylvania, minors between the ages of 14–18 must obtain a Work Permit.
Work Permits cannot be issued to children under the age of 14
Work Permits are issued by the school district in which the student resides. It is only valid for employment within the State of Pennsylvania.

Out Of State Employment—Any student seeking employment in another state, must secure a Work Permit from the school district in which the employer is located. For Maryland permits, visit : https://www.dllr.state.md.us/labor/wages/empm.shtml
Obtaining A PA Work Permit
The Work Permit Application Process:
Print off the form PDE-4565 from the KD High School Webpage or visit the SEMS School Counseling office for a paper copy
Print the name of minor only in Section A. The remaining sections will be completed by the issuing officer.
Complete Section B – Signature of parent, guardian or legal custodian & the address of parent, guardian or legal custodian.
Take the application and a copy of the evidence of age document (ie. Birth certificate, baptismal certificate, passport) to the School Counseling office. The School Counseling Office's Administrative Assistant serves as the issuing officer.
The issuing officer must witness the minor’s signature at“Signature of Minor”
The minor will be instructed that they are to keep the blue permit in their possession until their 18th birthday. The permit is transferable to any PA employer. It may be shown to the employer to be photocopied but must be returned to the student.

In the summer, work permit request must be made directly at Kennard Dale High School. Please call in advance to schedule a time to complete the process with a KD staff member.
