This form is to be used for a second or third sport season during the 2025-2026 school year IF the athlete was NOT injured, ill, or required medical attention outside of Serra Catholic athletic training since their full physical packet was filled out.   This page also is required if the full physical packet was filled out earlier than 6 weeks prior to the start of the season the student wishes to participate in (Example:  If Sarah only runs track and turns her physical packet in during September, she needs to fill out this form no earlier than 6 weeks prior to the start of the actual track season.)
