All necessary info is outlined on the 1st page (introductory letter).

Fall sports physicals are due to Ms. DeSalvo or the main office by August 1, 2026.

Winter and spring sports physicals are due 1 week prior to the start of their seasons.

Physicals cannot be PERFORMED OR CERTIFIED before May 1, 2026.

CERTIFICATION is fairly new.  Please see the introductory letter for more info/clarification.

Pages 8 and 9 must be signed by BOTH parent and student athlete.
