Graduation Application & Survey

Please skip this section if you graduated in December 2022.

This application/survey is a requirement for all graduating students (per the Graduation Policy). This application/survey also indicates how your name will be printed on your diploma; therefore, please ensure correct spelling and capitalization. Errors on your part will incur a $50 reprint fee and there is no guarantee the reprinted diploma will arrive in time for the ceremony.

ABHES Authorization of Attestation Form and Release of Information

Please skip this section if you graduated in December 2022.

As part of the college's accreditation, the reporting of placement data for all of our graduates is required. In the coming weeks, you will receive an email to your sentara.edu account via DocuSign to complete and return to the college electronically. The form has been emailed to your sentara.edu email accounts.

The form deadline is March 15, 2023.
