Q: What pages need to be filled out in the document?
A: Unless it is a recertification, the entire document must be completed. It is a typical mistake that people do not fill out a portion of the form. This can delay the approval process from the athletic office and prevent participation. Be sure to complete pages 1-9 of the form and submit them accordingly. (Page 10 must be completed for wrestlers)
Q: Who needs a completed PIAA form to participate in sports at STHS?
A: Every student who is participating in school sports (other than managers) needs to have this completed and approved by the athletic office prior to the start of the season.
Q: Where do I submit the form once it is completed?
A: The form can be dropped at either the athletic office or the front office with direction for it to be sent to the athletic office.
Q: Can my child still participate in the sport if they get the physical and form squared away after the start of the season?
A: This is ultimately at the discretion of the coach. However, if your child misses a mandatory tryout, it makes it near impossible to have them join the team later in the season.
Q: If my child played a spring sport last year that required the PIAA form and we completed it then, does that carry over for the fall season because it is the same calendar year?
A: No it does not. A PIAA certification or recertification from a previous year is essentially expired with the end of the school year.
Q: What is a recertification?
A: A recertification is a signature process on behalf the guardian asserting that there has been no notable change in the physical state of the athlete since the time of the original PIAA certification process from that year. This assertion on behalf of the guardian is completed by filling out pages 8 and 9 of the PIAA certification form and submitted to the athletic office for approval.
Q: When is a recertification required?
A: After an athlete participates in a sport in either the fall of the winter, any sport played in a subsequent season requires the recertification process or if there has been an injury to the athlete that requires a clearance to return to activity.
Q: Does the recertification need to be conducted with a doctor?
A: If there has been injury during the current season that required a clearance to return to activity, a doctor is required to sign the recertification. If no such injury is the case, then a guardian signature is sufficient.
Q: Where can a get a PIAA physical done?
A: A PIAA physical can be completed by an appropriately licensed doctor. The doctor can be from your family practice, a minute clinic, cvs clinic, urgent care or patient First. However, it must be signed by a doctor.
Q: What if my child's yearly well-check is not scheduled in an appropriate time that coincides with the start of the season?
A: Because the physical is a mandate, it would be necessary to secure the physical and completed paperwork at a Minute Clinic, Urgent Care or CVS that provides such a service.
