Transcript Request
Register with Parchment to send your transcript electronically or sign the release form below to have your transcript emailed or mailed.
**If you are under 18, a parent or guardian signature is required.**
To send transcripts electronically, go to the Parchment website and follow the directions to "Sign Up" or see Request my Transcript.
You can also DOWNLOAD and fill out the form below and email/mail request to the Counseling Secretary. *Please ensure you download the PDF first, as it will be fillable once downloaded.*
To obtain a transcript in person please stop into the Counseling Office between 7:30am-3:00pm to fill out the transcript release authorization form.
