Waivers and Forms

There are three waivers to sign. The one for Desert Adventures below (our rental company), the Medication Dispersement Form below (if needed) and the SBMS waiver on the Hub (will be sent out shortly).

Please have students AND parents print and sign the form above and return to Erin R by February 7, 2020.

Please print and sign the form above (if needed) and return to Lilli with labeled medications by Monday, March 9, 2020.
