My Child Has a Life-Threatening Allergy

If your child has a life-threatening allergy to food, insect stings, or another threat (such as medication or latex), you are already a pro at planning for their safety! Providing me with the information that I need (an allergy action plan, emergency medication, a release to communicate with your child's health care provider, & your experience/ perspective on what works best for keeping your child safe) to write a plan will help us confidently manage this need at school.

Since exposure to your child's allergen can potentially take place in the school setting (even with careful planning), training school staff to take quick action to recognize and manage allergy emergencies is a priority!

I use the video created by FARE (Food Allergy Research & Education)- available for free in the link at the left- to educate NFS teachers & staff on allergy emergencies.

Do you have a babysitter, family member, daycare provider or other individual who needs training? Share this link with them, too!

Let's make a plan to manage your child's allergy!

Step ONE: Please contact me by phone at 603-436-0910 ext. 4552 to discuss your child's needs. I will ask you to complete the Food Allergy Questionnaire (see below) with me over the phone if your child has a food allergy

Step TWO: Print & complete a Release of Information (or request one to be mailed/ sent home) to give permission to communicate and coordinate with your child's health care provider (I can fax required forms directly to your child's health care provider from my office and you can sign it when you bring medication)

AND/ OR

Print the preferred district Allergy Action Plan and take it to your child's health care provider. The healthcare provider should complete the form and you, the parent/ guardian, should sign it prior to returning it

Step THREE: If your child requires changes or substitutes to the food items served in the NFS kitchen in order to avoid a food allergen, print the Special Dietary Medical Statement and take it to your child's health care provider. If you have already signed a release of information, I can fax this form directly to the health care provider

Step FOUR: Bring a supply of medication to me at school. Prescription medications must be in a pharmacy labeled container. Over-the-Counter medications must be in an unopened original packaging.

Student's with life-threatening allergies may need additional planning or accommodations to be safe at school & while accessing school-sponsored events. For more information on Section 504, please check out that section of my Virtual Office!

Don't forget: If your child uses the school bus or an afterschool program, please be sure to make them aware of the allergy. You may sign additional releases to allow me to share planning information as necessary!
