Immunization Information

I want to thank you in advance for your efforts to get me your child's immunization information prior to the start of school!

I know it can be challenging to get to the doctor's office to pick that paperwork up- please remember that I welcome faxed immunization records at 603-427-2335! With a call to the healthcare provider office, you may be able to arrange for that fax without any unnecessary trips!

Every year, the State of New Hampshire publishes an update of the requirements for student immunizations (see the link on the left).

These immunizations are the required immunizations that all students of NH public schools are expected to have.

As the NFS Nurse, my job is to get a copy of your child's immunization record from their health care provider's office and keep it on file in their Student Health Record.

In late fall, I send a report to the State of New Hampshire regarding Immunization Requirement Compliance at NFS. I do not share names- just the number of students enrolled and the number of students in compliance with the requirements.

My goal is 100% COMPLIANCE


Luckily, there are 4 ways to put your child in compliance:

  1. Provide me with a copy of the complete immunization record from their health care provider's office

OR

  1. Provide me with a copy of their partial immunization record AND a note from their health care provider if they are unable to complete immunizations because of a medical problem (such as an allergy to an ingredient in the vaccine)

OR

  1. Provide me with their partial immunization record (if they have any) AND the completed Certificate of Religious Exemption

OR

  1. Provide me with their partial immunization record AND (if your child is on a catch-up schedule for immunizations) a note from their health care provider containing the appointment date for their next scheduled vaccination (don't forget to bring me the updated immunization record after they attend that appointment, please!)
