Log Entries

For safety and mandatory reporting, all schools should add this information for students only. Even if your school uses another software for this use, the data must also be entered in PowerSchool.

Adult offenses (by administrators, teachers, staff) are not recorded in PowerSchool.


It is the district's responsibility to :

  • Report all incidents to the DOE (Civil Rights Data Collection)
  • Communicate a pattern of issues to each school the student attends.
  • Report a pattern of offenses when s/he applies to the US Military, workforce, or other schools.
  • Prevent security issues throughout our district and others


What should you enter?

Add log entries for all disciplinary actions (for both victims and perpetrators of the issue). The district's new Behavioral Intervention Team is working on guidelines for this, but the following should definitely be entered. Remember that we are looking to establish patterns of behavior and will be used in each school that the student attends. It's important for Middle and High Schools to have a thorough history of student behavior.

  • Bullying
  • Cyberbullying
  • Harrassment (gender, race, orientation)
  • Violence
  • Restraints/Seclusion: Includes putting a student in a room as a "time out". This must be reported to the state.
  • Discipline/Safety Issues


To Create Log Entries

  1. Select student
  2. On left under Administration, click Log Entries
  3. Click New
  4. Fill out all applicable fields.
  • Be sure to enter the Consequence field.
  • Duration (Actual): Enter full days (no decimals)

5. Remember to check School Safety Category if the discipline relates to Harrassment, Bullying, Cyberbullying, Student Discipline/School Safety, or Restraints/Seclusion (meaning the school needed to use these measures). This information is used for the School Safety Survey run by each principal.

    1. Check off Use for Safety Survey
    2. Select Category
    3. Select applicable details that appear below.
    4. Then click Get New Incident ID.

6. Check Civil Rights Data Collection information in rare circumstances as they apply.

7. Click Submit


Notes: We will have a meeting on CRDC Data (Civil Rights Data Collection). This will be for anyone entering disciplinary data.
