NH STATE LAW IMMUNIZATION REQUIREMENTS
DTP/DT/DTaP/Td/Tdap
- 6 Years of age and younger: Four or five doses, with the last dose given on or after the 4th birthday.
- 7 years of age and older: Three or four doses, with the last dose given on or after the 4th birthday.
- Grade 7-12: 1 dose of Tdap is required for entry into 7th grade. A Tdap vaccine given on or after the 7th birthday meets the school requirement for 7th grade.
POLIO
- Kindergarten – Grade 7: Three to four doses, with one dose on or after the 4th birthday and the last two doses separated by 6 months.
- Grades 7-12: Three doses, with the last dose given on or after the 4th birthday; or four doses regardless of age at administration.
- If a combined IPV/OPV schedule was used, four doses are always required.
MEASLES-MUMPS-RUBELLA (MMR)
- Grades Kindergarten – 12: Two doses, at least one on or after the first birthday.
HEPATITIS B VACCINE
- Grades Kindergarten – 12: Three doses at acceptable intervals.
VARICELLA (CHICKEN POX) VACCINE
- Kindergarten – 10: Two doses or laboratory confirmation of chicken pox disease required.
- Grades 11 – 12: Two doses, or history of chicken pox disease in writing.
- In all grades the first dose of varicella must be on or after the first birthday
