Starting Kindergarten or Universal Transitional Kindergarten (UTK) is an exciting milestone! To help your child get off to a healthy and successful start, SDUSD requires the following health-related documents and screenings.
Students entering Kindergarten (if not previously submitted) must provide proof of a dental exam conducted within the last year - which must be turned in by April of their first year.
📄 Kindergarten Oral Health Assessment Form
📄 Report of Medical Examination for School Entry
California law requires all students to be up-to-date on state-mandated immunizations before starting school. Please provide:
Complete immunization record
Updates as your child receives new vaccines
📄 Preschool Immunization RequirementsÂ
📄 K-12 Immunization Requirements
Please Note: UTK students registering for 2025-26 who are three years old at the time of enrollment must meet the preschool requirements to enroll. Those who are four years old at the time of enrollment must meet the K-12 requirements to enroll.Â
SDUSD does not require children to be fully toilet trained to attend UTK - per the California Department of Education guidelines. We understand that young children develop toileting independence at different rates, and we are committed to supporting students with care and respect.
Here’s what families should know:
Accidents Happen: Occasional accidents are expected at this age. Please send a labeled change of clothes in your child’s backpack daily.
Privacy and Independence: Staff will encourage independence and provide verbal guidance.
Support Plans: If your child needs regular toileting assistance or has a developmental or medical condition that affects toileting, please contact the Case Manager A care plan or Individualized Education Plan (IEP) may be developed to support their needs.
Supplies: If your child wears pull-ups or requires specific hygiene supplies, please coordinate to bring these supplies daily or have a stock set at school.Â
Please see the SDUSD Toileting Toolkit for more information and guidance for families
