Immunizations

Kindergarten students entering Nokomis Elementary School must have written proof of the following immunizations at the time of registration:

4 polio

5 DPT

2 mumps

2 Rubella

2 Measles

2 varicella

3 Hepatitis B

Fifth grade students are required to provide proof of receiving a TDAP booster vaccine when they turn 11 years old. Students will not be permitted to start the sixth grade without proof of this vaccine. Students that have not yet turned 11 will be tracked, and must receive the vaccine when they turn 11.
