Google Forms
We will spend 20 minutes to complete the activity in this section.
What is it?
Google Forms is a free, browser-based, online survey/data collection tool in GSuite. It's a great way to enhance paper checklists!
How is it like a checklist?
- Drop-down list for names of students
- Curriculum expectation/look-for pre-populated
- Checkboxes for level
How is it better than a checklist?
- No need to sort through many papers/lists
- Open question for additional comments... but we can use voice-to-text!
- All data goes into a spreadsheet for easy sorting, referencing later
Taking Checklists to the Next Level!
One of the types of questions available on Google Forms is a "file upload" - which allows you to attach a file to your responses. On a mobile device, this file could be a photo or video, taken in the moment!
This can be a photo of the student's work as you speak with them, a photo/video of students in action, or a video that captures a conversation.
When completing the form on a mobile device, when you go to add a file, click on "select files from computer." An option should come up to take a photo or video right then and there.

- Add your students' names in a drop-down list

Go into either your Google Drive, or go to forms.google.com, and try your hand at creating a Google Form that works for monitoring a conversation/observation for an upcoming assessment.
