If you would like to select a birthday book, please return the form with $15.00 to the library.

Payment: We take cash in exact change or checks made to Joe Lee Johnson, and we can accept online payment. The item charge online is $13.50, but you will be charged $1.50 for an online payment fee, making the total $15, like the cash payment.
We will invite you to the library to choose your birthday book. On the next available Broadcast day, your photo will appear on the JLJTV news with your book!
