Each student enrolled in a Deerpark Choir class should complete their enrollment by submitting the Enrollment Fee for their class, along with the Choir Enrollment Form and Universal Permission Slip linked below! Please make sure to fill out the forms in their entirety. All forms and fees are due by September 15th, 2025

Sign this form once for the whole year! A list of dates covered by this permission slip is attached. Printed copies of this form will be distributed in classes.
Our district now has the ability to accept electronic payments!!!! Please follow the link to RRISDpay.com and select Deerpark Choir to make any and all choir payments this year. You will receive an automatically generated email receipt once your payment is received through the electronic "government window" system .
Any payments madeCheck should be submitted directly to the lockbox located in your students' choir classroom. Please make any checks out to Deerpark Choir or DMS Choir. The payment must be submitted in an envelope marked with the students full name, class period, and enclosed amount. You will receive an email receipt for cash or check payment once your director completes the deposit and logs your payment into our manual payment records
