No appointment is required for students who have previously registered for the Rocky Point UPK program. 

Please scan, attach and email all the below documents to:

If you are unable to scan and attach the below documents to an email, please call Ms. Agresto at 631-849-7244 to schedule an appointment. Please do not take photos of the documents and email the photos.

If your child is entering Kindergarten and has NOT attended a Rocky Point UPK or CPSE program, you must complete the full registration packet. Please click here.

Required Documentation:

1. Proof of residency

Provide One of the Following Items:

• Lease

• Deed

• Mortgage Statement

• Landlords’ Affidavit of Occupancy (if applicable) 

• Other Affidavits or documents (if applicable) 

Provide Two of the Following Items:

• Pay stub

• Electric bill

• Voter Registration

• Water bill

• Driver's License/Non-Driver ID Card

• Cable bill

• Bank Statement

• Gas bill

• Car Insurance/Registration

• Other official document

2. Student Registration Form / Formulario de inscripción de estudiantes

3. Emergency Contact Information Form / Información de Contacto en caso de Emergencia

4. Authorization to Use Computers and Photo Release Form / Autorización para usar computadoras y divulgación de fotografías

5. New York State School Health Examination Form (if exam submitted for UPK/CPSE entrance is older than 1 year) 

6. Email a PDF photograph of your child to jenniferagresto@rockypoint.k12.ny.us.
