You will need to fill out a registration packet (one per child) prior to your appointment as well as the required forms below. You can download the registration packet from the link below. If you are unable to download the packet, the District can mail the packet or arrange for pick-up. If the forms do not print correctly from your printer, do not fill them out; instead, please request a packet.
