Maxicare relaunched the Best Life Wellness Program this year to take a more active role in helping our members live healthier and more productive lives. The program gives free and exclusive benefits to members who have been diagnosed with or are at risk of hypertension, diabetes, and high cholesterol. These exclusive benefits are meant to prevent or manage these conditions.

Program enrollees can enjoy special benefits such as blood tests at home or at work, priority doctor and nutritionist teleconsults, and free medicines delivered to their homes or offices in Metro Manila.

ENROLL NOW IN FIVE EASY STEPS!

1. Fill out the enrollment form.

Enroll at https://bestlife.medgrocer.com/enroll/member-details, fill out your details, and answer our health screening questions. If you had HbA1c or Lipid Profile tests done in the last three months, you can upload your test results through the enrollment form.

Otherwise, you can get your tests from the comfort of your home via HomeCare or at your preferred Maxicare Primary Care Clinic (PCC). Just select your preferred laboratory and schedule upon enrollment.

2. Take your laboratory tests

If you select HomeCare for your baseline tests, our partner laboratory will reach out to confirm your appointment schedule.

If you selected PCC walk-in, visit your preferred PCC and present your Maxicare card and the laboratory request found in your email with the subject “Maxicare Best Life Wellness Program: Your Enrollment has been Received”.

Please note that fasting is required ten hours before your laboratory tests.

3. Attend consultations with doctors from Maxicare’s Primary Care Network

After you get your results, book a consultation with a doctor from Maxicare’s Primary Care Network to help you understand your results and receive your prescription.

You can consult a doctor through:

· Our Maxicare Corporate Clinic

· Maxicare Primary Care Clinic

· Maxicare Videoconsult (via the Doctor Anywhere app)

· Maxicare 24/7 Teleconsult

4. Request your FREE medicines

You can receive FREE medicines if you are enrolled in the program, have uploaded or completed the Best Life laboratory tests, and have a prescription from a doctor.

You can start receiving free medicines by uploading your prescription at https://bestlife.medgrocer.com/request

5. Get your medicines proactively refilled

After you place your first medicine request, you will receive 3-6 months’ worth of medicines covered by the Best Life Wellness Program. When your medicines are due for a refill, we will send you an SMS with the next steps for you to get your refills which may include:

· Updated laboratory tests through HomeCare or Maxicare PCC’s

· Completed doctor’s consultation from Maxicare’s Primary Care Network

· Updated prescriptions
