“Statement of Intent to Employ a Minor and Request for a Work Permit” (CDE Form B1-1): https://www.cde.ca.gov/ci/ct/we/documents/formb1-1.doc or you can pick up a form from the school's front office, Career Center, or Counseling department. 

1. Student must be enrolled at Carter High School

2. Student must attend school regularly, less than 5 absences per quarter

3. Student must maintain a GPA of 2.0 or higher

4. The student can not have any D's or F's in the previous semester

5. The student may not be suspended or expelled

6. The student must obtain and complete a Statement of Intent to Employ a Minor form B1-1

7. The student must submit the original completed Statement of Intent to Employ a Minor Form to Carter High School. No electronic copies will be accepted.
