Work Permit application

“Statement of Intent to Employ a Minor and Request for a Work Permit” (CDE Form B1-1): https://www.cde.ca.gov/ci/ct/we/documents/formb1-1.doc or you can pick up a form from the school's front office.

Work Permit Information:

Work permits expire every semester (First, second and Summer). Therefore, students must complete a new application (B1-1 Form) at the start of every semester. Students that meet the minimum requirements can drop off a completed work application- B1-1 Form at the school's front office. It will then be reviewed and processed. If approved you will be contacted via your Carter HS email. Please note there is 48-72 hour turnaround time on work permits, plan accordingly. If your work permit is incomplete or you do not meet our minimum requirements it will take longer to process and may be denied.

Work Permit Requirements:

  1. Student must be enrolled at Carter High School

  2. Student must attend school regularly, less than 5 absences per quarter

  3. Student must maintain a GPA of 2.0 or higher

  4. The student can not have any D's or F's in the previous semester

  5. The student may not be suspended or expelled

  6. The student must obtain and complete a Statement of Intent to Employ a Minor form B1-1

  7. The student must submit the original completed Statement of Intent to Employ a Minor Form to Carter High School. No electronic copies will be accepted.

Work Permit Procedure:

1.) Obtain employment and complete a “Statement of Intent to Employ a Minor and Request for a Work Permit” (CDE Form B1-1) at: https://www.cde.ca.gov/ci/ct/we/documents/formb1-1.doc or you can pick up a form from the school's front office.

Please make sure the form is correctly filled out before submitting it:

The student’s name, address, phone number, birth date and Social Security Number must be listed
- The parent's/ guardian's information and signature

The employer’s business name, address, phone number, and supervisor name and signature

The type of work the student will perform
- The student's, parent’s/ guardian's and the employer’s signatures must be on the form. The work permit CANNOT be processed if it is missing any signatures


2.) Bring the completed “Statement of Intent to Employ a Minor and Request for a Work Permit” (CDE Form B1-1) to Carter High School and drop it off at the front office. A completed work permit application includes the student, school, parent/guardian and employer information.
*If there is any information missing it will not be processed.


3) Once it is ready for pick up the student will receive an email to their Carter Gmail notifying them that it is ready. The student must be present when picking up the work permit.


Please note:
- Work Permits must be renewed every semester (first, second, and summer). You must complete a new work permit application (Form B1-1) every semester.

- If you are renewing a monthly work permit due to your grades or GPA please do not wait until the last minute to renew it. It is your responsibility to contact me and ask that your work permit be renewed.

- Please allow us 48-72 hours to process your work permit. If your work permit is incomplete or you do not meet our minimum requirements it will take longer to process and may be denied.
