Requirements

Option #1: Provide in total $20 to the Horizon NHS Fundraiser of your choice. Payments can be made through the bookstore. This can be given in monetary form or you may provide the amount in supplies to a school or charitable organization.

Option #2: Provide receipt (store receipt, a printout of online donation, paper written receipt, race bib, etc.) totaling $30 to an organization that you donated to on your own.  You must fill out the form below and submit paper documentation to your sponsor. Donations can NOT be combined with other service organization such as Key Club.

You may not collect food, clothing, toys, etc or donate items as a fundraiser
