Absent Form

If your child is absent, please scan this QR code and fill out the attached form. This will help us to easily communicate absences and the reasons for said absence. This helps to ensure the safety of all students. 

This form will also serve as an excuse note, unless the absence is longer than 3 days. In that case a doctor's note is still needed.
