Requirements:
Medical Results for x-ray and fecalysis (For Non-Food Handlers). This can be secured from any DOH-accredited laboratory/hospital
One 1x1 picture
Official Receipt for Health Permit
Fee is PHP 50.00
The certification can be issued the same day If the requirements are complete and there is no problem with the medical results.
