Registration

  • Records Request- Schools requesting transcripts and/or student records. Please attach request forms in email.

REGISTRATION REQUIREMENTS

In order to register a student, the biological parent or court appointed guardian MUST bring in the following documentation at the time of registration:

  • Birth Certificate
  • Student’s current immunization records
  • PROOF OF RESIDENCE (1 item needed)

MUST provide ORIGINAL document.

    • Settlement papers
    • Lease/Rental Agreement (Private party lease must be notarized)
    • Deed of Property
    • Mortgage Statement (within last 30 days)
    • Current Property Tax Bill
  • ADDITIONAL DOCUMENTATION (2 items needed)

MUST provide ORIGINAL document received within the last 30 days via US Postal mail which includes parent/guardian name and current address. Online documents and/or printouts will not be accepted.

    • Paystub
    • Non-cell phone bill
    • Utility bill
    • Credit card statement
    • Bank statement
    • Mail from government agencies (excluding PGCPS)
    • Hospital bill
    • W-2 form
    • Auto, home or health insurance policy
  • Student’s last report card or withdrawal packet
  • Parent/Guardian picture ID
  • If applicable, current IEP (Individual Educational Plan) or 504 plan
