Post-Internship Requirements
The Post-Internship Requirements are mandatory documents that must be submitted after the completion of the required internship hours for your academic program. These requirements serve as official proof that the internship was completed in accordance with CHED Memorandum Order No. 104, Series of 2017 and the policies of Pampanga State University.
Completion and submission of these requirements are necessary to officially complete the internship program and for the final evaluation of your practicum course.
Required Post-Internship Documents
All student interns are required to submit the following:
1. Daily Time Record (DTR)
Must reflect the actual dates and total hours rendered
Must be properly signed by the HTE Supervisor
Serves as official proof of attendance and internship hours
2. Monthly Accomplishment Report
A summary of tasks, activities, and learning experiences per month
Must be certified by the HTE Supervisor
Demonstrates the relevance of assigned tasks to your academic program
3. Internship Journal
A detailed narrative of daily or weekly activities, reflections, and learnings
Highlights skills gained, challenges encountered, and professional development
Used for academic assessment and monitoring
4. Internship Evaluation by the HTE Supervisor
A formal evaluation of the intern’s performance
Assesses competencies such as:
Work attitude and professionalism
Technical skills and knowledge
Communication and teamwork
Initiative and work ethics
Must be accomplished and signed by the designated HTE Supervisor
5. Certificate of Completion (Photocopy)
A detailed narrative of daily or weekly activities, reflections, and learnings
Highlights skills gained, challenges encountered, and professional development
Used for academic assessment and monitoring
Important Reminders
All documents must be complete, accurate, and properly signed
Incomplete or uncertified requirements may result in delayed internship completion
Internship hours will only be credited if completed within the approved deployment period
Submission and Coordination
Students are required to coordinate directly with their assigned SIPP Coordinator regarding:
Submission procedures
Deadlines and schedules
Required formats and templates
Failure to submit all post-internship requirements will result in non-completion of the internship program, regardless of the number of hours rendered.
