MEDICAL FORMS - All scouts, adults and adult leaders must have current medical forms on file with Pack. Please submit parts A, B1 and B2 yearly. These are for weekend campouts.

Please give forms to Mrs. Virginia.

Also include a copy of front & back of your health insurance!

Can Be Confusing!

Please view these examples to make sure you forms are filled out correctly and your scout, our yourself, receives the care you intended. Sometimes cell service at campgrounds are not that great and we have trouble reaching parents/guardians for permission for simple concerns.
