Emergency Information Guidelines

In the event of an emergency, please dial 911 immediately. Use the provided form to assess whether your specific situation warrants a 911 call. Be mindful that conditions such as unconsciousness, difficulty breathing, uncontrolled bleeding, head injury, and chest pain or pressure are indicative of situations that necessitate immediate attention and a prompt call to 911. Your swift and accurate assessment will contribute to a timely and appropriate response.

This form must be completed whenever an incident occurs on campus or during school sponsored or school related activities which involve students or members of your school community or a visitor to the school. The Incident Report should be completed by the OUSD employee who personally observes the incident, and/or who has the most relevant, first-hand knowledge of the incident. 

Complete this for if the incident involves a student (1) with an IEP, (2) having a behavior emergency and (3) the student was isolated, restrained, or required extensive supervision.
