It is a requirement by the Ottawa Carleton District School Board that any FORMER students who need to request a transcript, duplicate diploma, or other school record do so via this online form:

Request For Ontario Student Academic Records

*A processing fee will be applicable for former students who have been away from school for
more than one year.

For students currently attending Nepean High School, you can stop by the front desk in Student Services, or send a request to sandra.reid@ocdsb.ca - please include the reason you require a transcript. Note, there is NO CHARGE for your transcript while an active student, and for up to one year after leaving.
