- Take a picture of the signature page (make sure everything you’ve written is visible) and text it to Sara McAlister at 347-602-0241.

- Give the signed forms to your organizer.
- Mail the signed forms to Sara McAlister, 726 Broadway, 5th Floor, New York, NY 10003.

Youth Could Win a $100 Gift Card!
Youth organizer can win $100 gift card. If you return your assent form by the deadline (Check with the site leads or recruitment manager for deadline), you will be entered into a raffle to win one of 5 $100 gift cards. So if you complete your permission slip, whether you decide to participate in the study or not, you will receive one entry into the raffle. If your parent or guardian also completes a permission slip, whether they give permission or not, you will receive one entry into the raffle. If you return both permission slips, you get two entries into the raffle! Your odds of winning are about 1 in 40 if you submit both forms.
