This form ensures your concerns are heard, avoids duplicate submissions, and helps us prepare a focused agenda for each meeting. You can use it for anything related to curriculum, exams, academic resources, scheduling, or faculty/instruction feedback.

Please submit responses at least 48 hours before the monthly Academic Affairs meeting so they can be reviewed in time. Thank you!
