SOCIAL SECURITY FORMS

C-NS students who receive Social Security benefits may receive a form (SSA-1372-BK) in the mail that requires that our school complete a portion of the form. This form should be brought to the student's counseling office at C-NS High School. More information can be found at https://www.ssa.gov/schoolofficials/process_works.htm. 

You must complete Sections 1-7 and the Signature of Student portion BEFORE bringing  all pages of the form to your student's counseling office secretary.  The secretary will complete and sign the school's portion of the form and return it to you for mailing. We do not keep a copy of these forms so please make a copy if you wish to keep one. 

The form cannot be approved and signed by the counseling secretary unless sections 1-7 are complete and the Signature of Student portion has been signed and lists the student's Social Security number.
